1. Length of Session: Sessions will last approximately 60.45 or 30 minutes (depending on your chosen training schedule).

2. Punctuality: Client shall be attired as discussed below and ready to train at the time specified in point three (3). Failure to be prepared to train may result in a shortened workout or possible cancellation of the session under point six (6) below if Client is more than fifteen (15) minutes late. If client anticipates running late, he/she should contact the Training as soon as possible.

3. Attire: Client must wear comfortable workout attire, including, but not limited to, clean t-shirts, shorts, tights, sweats, and/or tracksuits. Athletic shoes must be supportive and functional. Please do not hesitate to ask your Trainer for advice on what type of clothing and shoes is appropriate.

4. Payment: All payment for Personal Training sessions are to be made two weeks in advance. It is CLIENT’S RESPONSIBILITY to pay his/her bill. Two weeks notice is required for any cancelation of service.

5. Rescheduling appointments: We understand that sometimes you may need to reschedule an appointment. We will do our best to accommodate your request. If your trainer is unable to find an alternative time, and your request is placed less then 24 hours prior to the appointment, you will be charged for the appointment (except under special circumstances or in case of emergency).

6. Canceling or missing appointment: If you miss a scheduled appointment or cancel with less than 24 hours notice, you will be charged for the scheduled training session (except under special circumstances or in case of emergency).

7. Trainer cancellation or missing an appointment: in the event of unexpected absence, your trainer will contact you as soon as possible and the trainer will reschedule the session with no penalty.

8. Medical freeze option: If you become ill or injured and have been advised by your doctor to temporarily discontinue training, you may freeze any remaining sessions for a period up to 3 months.

9. Terms and Conditions: The terms and conditions started herein will automatically be carried over to your new session upon expiry of the current package without the need for a new agreement to be signed and dated.

YOU ACCEPT THIS RISK AND ACCEPT FULL RESPONSIBILITY FOR YOUR EXERCISE AND TRAINING

Client certifies that:

a. He/she is physically capable of participating in a strength, flexibility and aerobic training exercise program and using the equipment associated with such training; and

b.he/she is over the age of eighteen(18) (if under the age a eighteen a guardians consent is required); and

c. he/she has either(|) had a physical examination and been given a physician’s permission to participate in this training program; or (ii) decided to participate in this training program without the approval of a physician.

This training program will involve physical and, as appropriate, use of certain equipment and machinery. Such activities may present a risk of injury or even death. Client voluntarily agrees to participate in these activities and to assume all risk of personal injury, death and property damage resulting from such activities, use of equipment, machinery or public or private facilities.
